Athletic Clearance
All athletes must have athletic clearance in order to be allowed to participate in cross country and/or track and field. In order to get athletic clearance, athletes must fill out a few forms. Physicals are good for one year before they need to be renewed. These are also on the Poway High School website:

1. Obtaining a physical evaluation
Athletes must fill first fill out a medical history form, and then have another form filled out by a physician. These forms can be found here:

2. Submitting the evaluation
Upon completing these forms, they should be submitted to the Athletic Office by the due date:
June 7 for Fall Season Sports
October 11 for Winter Season Sports
January 10 for Spring Season Sports
If you happen to miss one of these due dates, the forms can still be submitted. However, it will take a minimum of one week to be processed. All forms can be turned in to the Athletics Office during normal school hours.
